If You Need To Request Medical Records ~ There Is A Charge of $100

MEDICAL RECORDS REQUEST FORM

Remember that parents received a BLUE USB Flash Drive containing a complete copy of their child’s medical records when the practice closed.

 

  • The entire library of Medical Records for Partners in Pediatrics, PC has been archived.
  • To retrieve your child’s medical records, you will need to send written request for the documents to the post office box retained for the purpose of securing the documents.
  • If you are unable to print and mail the form provided, you may handwrite a letter containing the information in the form.
  • The FEE FOR RETRIEVAL plus Copying and Creating paper and/or digital copies of the child’s medical records, and Mailing or Faxing to the designated individual is $100 (one hundred US Dollars).
  • Please include payment with your request mailed to the practice post office box address above.
  • Please allow thirty (30) business days although it is unlikely that it will take that long to process the request.
